Liposarcoma Retroperitoneal Gigante: A Propósito de un Caso  [cached]

Abstract: Los tumores retroperitoneales representan un 0,07 a 0,2% de todas las neoplasias del organismo. Los liposarcomas, formas predominantes de sarcomas, se originan del mesodermo embrionario y pueden alcanzar grandes dimensiones en el retroperitoneo, presentándose en forma de masas voluminosas sin sintomatología específica.Las metástasis a distancia son poco probables; la mortalidad específica es del 40 al 50% a los 5 a os del diagnóstico y su pronóstico depende de su variedad histopatológica. La conducta es quirúrgica y consiste en exéresis radical del tumor; junto con la radioterapia y la quimioterapia como tratamiento paliativo. Se presenta un caso del liposarcoma retroperitoneal gigante bien diferenciado de células fusiformes; manifestado por distensión abdominal progresiva acompa ado de dolor tipo opresivo de moderada intensidad y el tratamiento efectuado, la resección quirúrgica radical.

Abstract: objectives: to describe a patient with dedifferentiated liposarcoma of the spermatic cord and to perform a bibliographic review. methods: we describe the case of a patient with dedifferentiated liposarcoma of the spermatic cord, and conducted an extensive search of literature in different databases that support the discussion of the case. results: a 65 years old male presented a right scrotal mass with progressive growth, and underwent excision of the mass (measured 34 x 22 x 17 cm and weighed 5786 grams). pathology reported dedifferentiated liposarcoma of the spermatic cord. this patient draws attention to the reoccurrence of papillary thyroid cancer. conclusions: paratesticular tumors are rare entities, derived from mesenchymal tissue. most of them are benign, but up to 30% could be of sarcomatoid type. their treatment is based on the extraction of the mass along with radical orchiectomy, but being a very rare disease is not clear whether the benefit of adjuvant chemotherapy and radiotherapy in these patients is beneficial.

Abstract: liposarcoma is a malignant tumor that have an embryologic origin from mesodermal tissue depending on fatty tissue. although liposarcoma is only 0.1% of all human neoplasms is the most common histology subtype of retroperitoneal soft tissue sarcomas. this tumor grows slowly. diffuse abdominal pain is its most frequent symptom and abdominal mass is the most common sign. aggressive surgical treatment is basic to get a complete resection and a local disease control. this objetive is difficult because of the large tumor size it gets in the retroperitoneal location and the multiorgan involvement that require the resection of a high percentage of contiguous organs. we report a case of a giant retroperitoneal liposarcoma presenting like continuous left hemiabdominal pain because the visceral compression. the right kidney was involved and suffering from renal vessels enlargement without renal function. there is a high probability of microscopic residual disease and a good follow-up of the patients is necessary as well as an adjuvant radiation therapy in some cases.

Abstract: Se presenta el caso de una mujer de 59 a os que había sido intervenida quirúrgicamente en 3 ocasiones por presentar nódulos en el cuello. La biopsia de 2 de ellos informaba que se trataba de un liposarcoma pleomórfico. Pasados 6 a os acude a consulta por un cuadro de dolor retroesternal alto y ligera disnea. La tomografía de tórax muestra una masa sólida en el mediastino anterior. Se realizó la exéresis de una tumoración de consistencia blanda, de 10 cm. Anatomía patológica informó la recidiva de un liposarcoma pleomórfico. La evolución posoperatoria en general fue satisfactoria. La literatura revisada muestra que el tratamiento quirúrgico es el más empleado en estos casos. Authors present the case of a woman aged 59 operated on in three occasions due to the presence of neck nodules. Biopsy from two of them informs that be about a pleomorphous liposarcoma. After 6 years she came to consultation by a picture of high retrosternal pain and slight dyspnea. Thorax tomography shows a solid mass in anterior mediastinum. We made exeresis of a 10 cm diameter soft solid mass. Pathological Anatomy Service reported on relapse of a pleomorphous liposarcoma. In general, the postoperative course was satisfactory. Literature reviewed shows that the surgical treatment is the more used procedure in these cases.
Lipoma gigante cervico -torácico. Abordaje mixto por cervicotomía y toracoscopia: Reporte de un caso

Abstract: Se presenta un caso de lipoma cervicotorácico gigante en paciente femenino de 45 a os de edad, quien consultó por aumento de volumen cervical, odinodisfagia y disnea. Una radiografía de tórax y tomografía computarizada reveló una masa gigante intratorácica bilateral, ocupando la cavidad torácica izquierda con extensión al mediastino anterosuperior y al cuello, desplazando la tráquea y la faringe hacia la porción anterior y derecha. La paciente se llevó a resección quirúrgica del tumor cervico-torácico, con examen histológico que confirmó el diagnóstico de un lipoma gigante, con peso de 475 gramos y medidas de 30 x 20 cm. Este es el lipoma cervico-torácico más grande documentado en la literatura moderna y con abordaje mixto cervicotomía más toracoscopia. Female patient 45 years old with giant cervicothoracic lipoma, that consulted by increase of cervical volume, odinodysphagia and dyspnea. An x-ray thorax and computerized tomography revealed a bilateral intrathoracic giant mass, occupying the left thoracic cavity extending to the anterosuperior mediastinum and the neck, causing displacement of the trachea and the pharynx towards the previous and right portion. The patient took to surgical resection of the cervical thoracic tumor, with histological examination that confirmed the diagnosis of a giant lipoma, with weight of 475 grams and measures of 30 x 20 cm. It is largest cervical thoracic lipoma documented in modern literature and with cervicotomy and thoracoscopic approach.

Abstract: here, we describe the case of a 51-year-old female with mediastinal liposarcoma. liposarcoma is the most common malignant mesenchymal neoplasm in adults, although a mediastinal location is extremely rare. it has a large volume and varied histologic subtypes. it is characterized by the compression of neighboring structures. computed tomography and magnetic resonance imaging provide useful data for diagnosis. tissue biopsy and histological typing are very important in determining the treatment and are needed for the final diagnosis. radiotherapy and chemotherapy are ineffective treatment modalities. according to the literature, surgical resection is the treatment of choice. long-term follow-up evaluation is indicated since there is a high rate of recurrence.

Abstract: most mediastinal tumors are incidentally diagnosed because they are asymptomatic. the aim of this paper is to report on the case of a 29-year-old male patient, who had an increase in cardiac area after blunt thoracic trauma. he was submitted to thoracotomy when a large mediastinal mass was found. the histopathological diagnosis was a mature mediastinal teratoma.

Abstract: Background: Surgery is the unique possibility of cure of survival with life quality for patients with tumor or mediastinal mass. Objective: To describe the results of surgical procedures in patients with mediastinal tumor. Methods: Descriptive, retrospective study of 29 patients operated after being diagnosed with mediastinal tumor in a period of 17 years (1986-2002). Studied variables were: tumor′s locus in the mediastinum, tumor nature, histological diagnosis and strategies regarding surgical approach. Findings: There was a prevalence of benign tumors (81,8%) and the most frequent locus was anterior mediastinum. Conclusions: These findings agree with previous studies, mainly regarding tumors′ locus and nature. Fundamento: La cirugía es la única posibilidad de curación o de supervivencia con calidad de vida, de los pacientes con diagnóstico de tumor o masa mediastinal. Objetivo: Describir los resultados de los procedimientos quirúrgicos en pacientes con diagnóstico de tumor de mediastino. Métodos: Estudio descriptivo y retrospectivo de una serie de 29 pacientes operados tras el diagnóstico de tumor de mediastino, en un período de 17 a os (1986-2002). Las variables estudiadas fueron: localización de la tumoración en el mediastino, naturaleza del tumor, diagnóstico histológico y estrategias en cuanto al abordaje quirúrgico. Resultados: Predominaron los tumores benignos (81,8 %) así como la localización en el mediastino anterior. Conclusiones: Los resultados concuerdan, en gran medida, con otros estudios realizados sobre este tema, pero sobre todo en lo referente a la naturaleza y localización de los tumores.
