Aferi o dos equipamentos de laser de baixa intensidade Calibration of low-level laser therapy equipment  [cached]

Abstract: CONTEXTUALIZA O: A laserterapia de baixa intensidade (LBI) vem sendo cada vez mais utilizada, porém ainda n o há consenso na literatura quanto ao tempo em que os equipamentos devem ser submetidos à aferi o ou calibragem. OBJETIVO: Analisar a potência média real (PmR) dos equipamentos de LBI na regi o da Grande S o Paulo. MéTODOS: Para análise dos equipamentos, utilizou-se um potenci metro (Lasercheck), próprio para aferi o de equipamentos contínuos, o qual foi programado com dados referentes ao comprimento de onda do laser a ser avaliado, obtendo-se assim a PmR emitida. Os equipamentos foram analisados de duas formas: uma, com o LBI desaquecido, e outra, após 10 minutos de uso (aquecido), sendo que três análises foram feitas para cada condi o. A caneta emissora foi acoplada ao potenci metro, o qual fornecia a PmR emitida pelo LBI. Todos os dados e informa es referentes à aplica o do laser foram coletados por um questionário respondido pelos responsáveis. RESULTADOS: Os 60 equipamentos avaliados mostraram déficit na PmR com os equipamentos desaquecidos e aquecidos. A análise estatística (ANOVA) mostrou diminui o significativa (P<0,05) da PmR aferida em rela o à potência média do fabricante (PmF). Em média, a dose mais empregada nas clínicas foi de 4 J/cm2, tendo os efeitos de cicatriza o e anti-inflamatório como os mais desejados. Segundo a World Association for Laser Therapy (WALT), para atingir esse efeito, necessita-se de 1 a 4 J de energia final, sendo que apenas um dos 60 aparelhos conseguiria atingir a janela terapêutica preconizada. CONCLUS O: Os equipamentos de LBI apresentam um déficit acentuado na PmR, o que mostra uma desordem na utiliza o desse recurso. Neste estudo, observou-se a necessidade de aferi o periódica dos aparelhos de LBI bem como melhor conhecimento técnico dos profissionais envolvidos. Abstract: background: despite the increase in the use of low-level laser therapy (lllt), there is still a lack of consensus in the literature regarding how often the equipment must be calibrated. objective: to evaluate the real average power of lllt devices in the greater s?o paulo area. methods: for the evaluation, a lasercheck power meter designed to calibrate continuous equipment was used. the power meter was programmed with data related to the laser's wavelength to gauge the real average power being emitted. the lllt devices were evaluated in two ways: first with the device cooled down and then with the device warmed up for 10 minutes. for each condition, three tests were performed. the laser probe was aligned with the power meter, which provided the real average power being emitted by the lllt device. all of the data and information related to the laser application were collected with the use of a questionnaire filled in by the supervising therapists. results: the 60 devices evaluated showed deficit in real average power in the cooled-down and warmed-up condition. the statistical analysis (anova) showed a significant decrease (p<0.05) in the real average power measured in relation to the manufacturer's average power. on average, the most common dose in the clinics was 4 j/cm2, and the most desired effects were healing and anti-inflammatory effects. according to the world association for laser therapy (walt), 1 to 4 j of final energy are necessary to achieve these effects, however only one device was able to reach the recommended therapeutic window. conclusion: the lllt devices showed a deficit in real average power that emphasized a lack of order in the application of this tool. the present study also showed the need for periodical calibration of lllt equipment and a better technical knowledge of the therapists involved.

Abstract: Objetivos: verificar histologicamente a cicatriza o de feridas cutaneas experimentais em Rattus norvergicus quando empregado o laser de baixa potência. Métodos: foram utilizados 45 animais machos, distribuídos em três grupos conforme a terapêutica adotada (n o tratados, tratados com laser na dose de 4 J/cm2 e tratados com laser na dose de 8 J/cm2), os quais foram divididos em três subgrupos conforme o período de eutanásia (7, 14 e 21 dias). Foi removido um fragmento cutaneo de 2 cm de diametro do dorso de cada animal e a cada 24 horas foi irradiado laser nos fragmentos dos grupos tratados, com a dose estabelecida previamente. Resultados: aos sete dias após o experimento, os grupos irradiados com laser de baixa potência apresentaram uma reepiteliza o mais eficiente quando comparados ao grupo n o tratado. Além disso, os grupos tratados com laser apresentaram uma deposi o de colágeno mais acentuada quando transcorridos 14 dias. A dose ajustada em 4 J/cm2 foi mais efetiva. Conclus es: o laser de baixa potência otimizou o processo de cicatriza o de feridas cutaneas experimentais em Rattus norvergicus. A dose ajustada em 4 J/cm2 foi mais efetiva. Aims: To assess histologically the healing of experimental skin wounds in Rattus norvergicus using low-power laser. Methods: Forty-five males were divided into three groups according to the adopted therapy (untreated, treated with laser at a dose of 4 J/cm2 and treated with laser at a dose of 8 J/cm2), which were divided into three subgroups according to time of euthanasia (7, 14 and 21 days). A fragment of 2 cm in diameter of skin was removed from the back of each animal, and laser was irradiated every 24 hours in the fragments of the treated groups, with the previously established dose. Results: At seven days after the experiment, the low-power laser irradiated groups showed a more efficient reepithelialization when compared with the untreated group. Furthermore, the groups treated with laser showed a more marked deposition of collagen at 14 days. Dose adjusted in 4 J/cm2 was more effective. Conclusions: Low-power laser has enhanced the healing of experimental skin wounds in Rattus norvergicus. The adjusted dose in 4 J/cm2 was more effective.

Abstract: o objetivo do estudo foi realizar uma revis?o de literatura a respeito da terapia com laser de baixa potência e sua rela??o com as fases iniciais de reparo. foram analisados 22 artigos, observou-se a utiliza??o de diferentes doses e comprimentos de ondas (632,8 a 904 nm). nos estudos in vitro foram utilizadas doses entre 2,2 e 16 j/cm2. a dose de 5 j/cm2 tem sido apontada como responsável por mudan?as significativas in vitro, porém a dose de 16 j/cm2 promove efeito inibitório sobre o crescimento celular em culturas. em estudos in vivo, envolvendo animais foram utilizadas doses entre 0,04 a 21 j/cm2. para estudos em humanos foram utilizadas doses entre 1,8 a 16 j/cm2. conclui-se que a terapia com laser de baixa potência exerce efeitos antiinflamatórios importantes nos processos iniciais da cicatriza??o: redu??o de mediadores químicos, de citocinas, do edema, diminui??o da migra??o de células inflamatórias e incremento de fatores de crescimento contribuindo diretamente para o processo de reabilita??o tecidual. porém, a falta de padroniza??o dificulta a escolha de parametros ideais.

Abstract: objective: to perform an in vitro bacteriologic study to evaluate the effect of two types of low level laser (lll) on different bacterial populations usually present in post-traumatic wounds. methods: swabs were prepared directly at the infection site of patients hospitalized with chronic post-traumatic osteomyelitis. isolated bacteria were acinetobacter baumanii complex, escherichia coli, haemophilus influenzae, klebsiella pneumoniae, pseudomonas aeruginosa, salmonela sp, serratia sp, and staphylococcus aureus. the material collected was seeded in agar-blood medium with a sterile loop, using 30 petri dishes for each germ. two lll devices were used: ibramed laser pulse #01189, with 15w/904nm for 200 seconds, and phisiolux dual bioset #9909001, with 20w/904nm for 230 seconds. in groups i (n = 10) and ii (n = 10), bacteria were irradiated with laser. group iii (n = 10) was the control group and was not irradiated. bacteria in groups i and ii were submitted to radiation in a laminar flow chamber that was previously sterilized with uv rays, and the laser was directly, centrally, and perpendicularly applied to the bacteria cultivation surface, from a standard distance of one centimeter, through an orifice made in the lid of the dishes. bacterial growth was analyzed 12 and 24 hours after the irradiation. results were statistically processed using the non-parametric test of kruskall-wallis, with a significance level p < 5%. results: a similar behavior was seen in the bacterial population of the three groups studied after 12 and 24 hours of irradiation with the two types of lll, and there was no statistically significant difference in the bacterial growth between groups i and ii and between these two groups and group iii (control). conclusion: in the conditions of this study, the effect of lll showed to be innocuous for the increase in the number of units forming bacterial colonies, in the doses used in this study, as an adjuvant for the wound healing process, even under co

Abstract: the authors report a case of a 89 years-old woman with mucocutaneous leishmaniasis and previous diabetes mellitus and high blood pression, who had been treated with allopurinol for 10 months without healing of lesions. afterwards, she has been treated with meglumine antimonate, "glucantime" for 4 days, with a total dose 2,380mg of sbv, but developed cardiacs side effects and hypokalemia, hence the treatment was wihtdrawn. however, this patient developed total clinical regression of lesions, in spite of she has been recieved low dose of this drug .

Abstract: Os autores relatam um caso de leishmaniose cutaneo-mucosa em uma paciente de 89 anos, diabética e hipertensa, tratada inicialmente com alopurinol por 10 meses n o havendo cicatriza o das les es. Posteriormente, recebeu antimoniato de N-metil glucamina (glucantime) por 4 dias, na dose total de 2.380mg do Sb v, mas desenvolveu cardiotoxicidade e hipocalemia, sendo suspenso o tratamento, entretanto, evoluiu com regress o clínica total das les es, apesar de ter recebido pequena dose desta medica o.
Preditores de viabilidade em pacientes com resposta negativa à ecocardiografia de estresse com dobutamina de baixa dose

Abstract: background: low-dose dobutamine stress echocardiography is specific for predicting reversible contractility dysfunction, but its sensitivity is lower than ideal. objective: we sought to explore the predictors of myocardial contractile recovery following revascularization, in patients with no viability by low-dose dobutamine stress echocardiography. methods: we prospectively enrolled 30 consecutive patients with significant coronary stenosis/occlusion amenable for revascularization, regional wall motion abnormality in the distribution of the affected artery and absence of viability by low-dose dobutamine stress echocardiography. they underwent resting 99mtc-sestamibi imaging study, and then underwent successful coronary revascularization. follow-up echocardiography was performed 3 months later. patients were classified into 2 groups: group 1: with evidence of myocardial contractile recovery after revascularization at follow-up echocardiography and group 2: with no such evidence of recovery. the two groups were compared with respect to patients’ clinical, echocardiographic and scintigraphic data. results: the mean age was 52.3 ± 5.9 years, with 97% being males. the percentage of total 99mtc-sestamibi uptake was significantly higher in group 1 as compared to group 2 (p < 0.01), and it was the strongest independent predictor of myocardial contractile recovery at 3-month follow-up by multivariate regression analysis. receiver operating characteristics curve revealed that a cutoff value of the percentage of total 99mtc-sestamibi uptake of 72% best predicted myocardial contractile recovery, with a sensitivity of 100% and specificity of 95.7%. conclusion: in patients with no viability by low-dose dobutamine stress echocardiography, the percentage of total 99mtc-sestamibi uptake independently predicted myocardial contractile recovery following coronary revascularization.

Abstract: purpose: to describe the maternal and perinatal outcomes after the use of 12.5 μg of sublingual misoprostol for labor induction in women with term pregnancy and a live fetus. methods: we conducted a multicenter, open and non-randomized clinical trial during the period from july to december 2009. we included 30 pregnant women with an indication for labor induction at term, carrying a live fetus, with a bishop score of six or less, cephalic presentation, estimated fetal weight of less than 4,000 g and an amniotic fluid index greater than five. we excluded women with a previous uterine scar, non-reassuring fetal status, congenital anomalies, multiple pregnancy, intrauterine growth restriction, genital bleeding, and contraindications of vaginal delivery. a tablet of 12.5 μg sublingual misoprostol was administered every six hours, until the beginning of labor, with the maximum of eight doses. results: labor was successfully induced in 90% of pregnant women. the mean interval between the first dose and the onset of uterine contractions and delivery was 14.3±11.7 hours and 25.4±13 hours, respectively. the frequency of vaginal delivery was 60%. uterine tachysystole occurred in two pregnant women, being reversed in both cases without the need for cesarean section. meconium-stained amniotic fluid was observed in four patients, and an apgar score of less than 7 at five minutes in only one newborn. conclusion: maternal and perinatal outcomes were favorable after induction of labor with sublingual misoprostol at a dose of 12.5 μg every six hours. however, controlled trials are needed to compare this regimen with other doses and routes of administration.
Baixa dose de methotrexate comparado a placebo em osteoartrite de joelho

Abstract: objective: to evaluate the efficacy of low-dose methotrexate in the treatment of knee osteoarthritis. methods: 58 patients were separated into two groups in a prospective, double-blinded, placebo-controlled, randomized 4 months study. group 1 received a 7,5 mg weekly dose of methotrexate whilst group 2 received placebo. western ontario and mcmaster universities osteoarthritis index (womac), lequèsne algofunctional index and the visual analogue scale (vas) for pain were utilized as measurement of primary evaluation. the quantity of paracetamol used for analgesia served as a secondary evaluation. results: the patient?s average age was of 60.4 years in the group 2 and 61.8 years in group 1 (p = 0.43). in both groups, women predominated, comprising 93.1% in group 1 and 72.4% in group 2 (p = 0.08). there were no statistically significant difference between both groups regarding womac (p = 0.94), lequèsne (p = 0.87) and vas (p = 0.89. likewise, in the paracetamol consumption there was not statistically significant difference between both groups. however, there was tendency to increased consumption in the placebo group. conclusion: methotrexate provided neither symptomatic relief nor did it reduce the functional limitation when compared to placebo in knee osteoarthritis. methotrexate showed a tendency to decrease the consumption of paracetamol in knee osteoarthritis.
