Abstract: OBJETIVO: Avaliar o efeito sublingual do mononitrato-5 de isossorbida (MN5IS) e nitroglicerina (NTG) sobre o diametro luminal de artérias coronárias epicárdicas, press o arterial média e efeitos colaterais. MéTODOS: Cinqüenta pacientes foram submetidos a cateterismo cardíaco e cinecoronariografia, na condi o inicial e 5min após administra o sublingual de MN5IS grupo A (GA) ou NTG grupo B (GB). RESULTADOS: O diametro coronário de referência aumentou em ambos os grupos, sem significancia estatística entre os mesmos. Nos GA e GB foram demonstrados uma diminui o (1,66mmHg) e um aumento (0,79mmHg) na press o arterial média, respectivamente (p=0,123). N o foram observados efeitos colaterais com o uso destas drogas. CONCLUS O: MN5IS sublingual é uma alternativa à administra o de NTG durante cinecoronariografia e representa uma alternativa terapêutica para o tratamento de doen a cardíaca isquêmica.

Abstract: purpose: to evaluate the effect of sublingual isosorbide-5 mononitrate (ismn) and nitroglycerin (ntg) on luminal diameter of epicardial coronary arteries, mean arterial pressure and deleterious effects. methods: fifty patients were submitted to cardiac catheterization and coronary arteriography, at baseline, and 5min after sublingual administration of ismn, group a (ga) or ntg, group b (gb). results: reference vessel diameter increased in both groups, without statistical significance. in ga and gb, a decrease (1.66mmhg) and an increase (0.79mmhg) in mean arterial pressure, respectively, were demonstrated (p=0.123). there were no deleterious effects with the use of these drugs. conclusion: sublingual ismn is an alternative to administration of ntg during coronary arteriography, and represents a therapeutic alternative to ischemic heart disease treatment.

Abstract: Microcirculation plays a crucial role in the interaction between blood and tissue both in physiological and pathophysiological states. Orthogonal polarization spectral (OPS) and Side stream dark field (SDF) imaging are relatively new noninvasive methods that allow the investigation of mucosal sites, especially sublingual area, particularly in critically ill patients. OPS imaging has been validated against conventional capillary microscopy, results demonstrated that OPS images provided similar values for RBC-velocity and capillary diameter as those measured by conventional capillary microscopy. Despite some limitations, sublingual microcirculation has been considered as a possible surrogate measure for splanchnic blood flow. There are several areas in human medicine, where observation of sublingual microcirculatory bed has been carried out – different kinds of shock, cardiac arrest, effect of various treatments, drugs and extreme physiological conditions as well. Early detection of microvascular abnormality seems to be a key factor to start early therapeutic intervention in order to reverse microvascular dysfunction, to maintain efficient microvascular flow and to contribute to better clinical outcome. In experimental setting, observing sublingual microcirculation is an important part of any research focused on the role of microcirculation during various diseases models and to assess effect of different treatment modalities on microcirculation.

Abstract: Introducción: la literatura científica indica que dos de cada tres pacientes con dolor crónico sufren con cierta frecuencia exacerbaciones puntuales del mismo debido a diferentes causas, en ocasiones previsibles, y en otras inesperadas. Además del sufrimiento que conllevan, estos episodios constituyen un problema importante para el paciente pues generan ansiedad y a aden incapacidad funcional, lo cual se traduce en una mayor dificultad para controlar el dolor basal y una menor calidad de vida. En 1990 se acu ó en Estados Unidos el término "break-through pain", para definir a las exacerbaciones transitorias de un dolor oncológico, que está bien controlado con la utilización de opioides mayores. En el a o 2002, la Sociedad Espa ola de Oncología Médica (SEOM), la Sociedad Espa ola de Cuidados Paliativos (SECPAL) y la Sociedad Espa ola del Dolor (SED), establecieron un documento de consenso en el que asumieron el término "dolor irruptivo", para definir una exacerbación del dolor de forma súbita y transitoria, de gran intensidad (EVA > 7) y de corta duración (usualmente inferior a 20-30 minutos), que aparece sobre la base de un dolor persistente estable, cuando este se encuentra reducido a un nivel tolerable (EVA < 5) mediante el uso fundamental de opioides mayores. De forma general se acepta la existencia de tres tipos etiológicos de dolor irruptivo: dolor irruptivo incidental; dolor irruptivo idiopático o espontáneo, y dolor irruptivo por fallo de final de dosis. Objetivos: el objetivo principal de este trabajo fue estudiar retrospectivamente la efectividad del citrato de fentanilo sublingual en 180 pacientes tratados por dolor irruptivo en las Unidades de Dolor de Andalucía durante 1 mes. Como objetivos secundarios se buscó describir las indicaciones clínicas y características epidemiológicas de los pacientes con dolor irruptivo que se encuentran en tratamiento con citrato de fentanilo sublingual; conocer las pautas de administración del citrato de fentanilo sublingual en pacientes con dolor irruptivo; e investigar los posibles acontecimientos adversos que pueden asociarse al uso de citrato de fentanilo sublingual en pacientes con dolor irruptivo. Material y métodos: estudio observacional retrospectivo en una muestra de 180 pacientes, de los que 173 finalizaron correctamente el estudio. Los criterios de inclusión de los pacientes (debían cumplir uno de los dos siguientes) fueron: episodios de DI con EVA > 5 en las últimas 12-24 horas y/o efectos secundarios indeseables originados por el tratamiento actual del dolor irruptivo. Abstract: purpose: to compare the effectiveness and safety of sublingual misoprostol (25 μg) versus vaginal misoprostol (25 μg) (prostokos?) for labor induction with gestational age > 37 weeks and unripe cervices. methods: a randomized controlled clinical trial was performed at the maternidade monteiro de morais (cisam-upe), in recife - pe, brazil, from october 2003 to february 2004. one hundred and twenty-three women with gestational age > 37 weeks, bishop score <8, not in labor and with medical indication for interruption of pregnancy were included in this study. the women received randomly 25 μg sublingual misoprostol or 25 μg vaginal misoprostol every 6 h, not exceeding eight doses. in order to evaluate the differences between the groups, means, standard deviations, student's t-test, c2 trend and mann-whitney test were used. the statistical significance was considered to be 5%. results: there were no significant differences between the number of women with vaginal delivery in the sublingual group as compared with the vaginal group (65.5 vs 75.8%, p<0.22), or in the interval of time between the induction onset and delivery (24 h and 42 min vs 20 h and 37 min respectively, p=0.11). the two groups, sublingual and vaginal, also did not differ as to the hyperstimulation syndrome (1.7 vs 3.2%, p=0.95), meconium incidence (5.2 vs 4.8%, p=0.74), apgar score <7 at 5 min (3.4 vs 4.8%, p=0.98) and other adverse effects. conclusion: twenty-five micrograms of sublingual misoprostol every six h presented the same effectiveness and safety as an equal vaginally administered dose of this substance. sublingual misoprostol seems to be acceptable and is another option to be considered for labor induction.

Abstract: objectives: to determine effectiveness and safety of sublingual misoprostol in tablets of 25mcg, given every 6 hours for induction of labor in high-risk pregnant women hospitalized in two teaching hospitals in the northeast of brazil. methods: an open, non-randomized clinical trial was conducted, including 40 women with high-risk pregnancies hospitalized at "maternidade-escola assis chateaubriand" and "instituto materno-infantil de pernambuco". all of them had gestational age > 37 weeks, alive fetus with good vitality and bishop scores < 7. a tablet of 25mcg of misoprostol was administered every 6 hours, up to a maximum of 4, until active labor. statistical analysis was performed using the public domain software epi-info 3.2.2. results: active labor occurred in 100% of cases after misoprostol administration. the mean (±sd) induction-to-labor interval was 4.8±3.8 hours. interval from induction-to-delivery varied from 8 to 31 hours with 95% of the deliveries occurring in the first 24 hours with 75% of vaginal deliveries. the frequency of tachysystole was 12.5%. the women did not present relevant side effects neither were there any neonatal complications. conclusions: the sublingual tablet of misoprostol of 25mcg was shown to be effective for induction of labor in high-risk pregnant women. the efficacy and acceptability of this new route should be compared to vaginal administration of misoprostol in future prospective randomized clinical trials.
