This article presents a clinical case illustrating a 65-year-old patient with MVP associated with characteristic echocardiographic signs, including mitral annular disjunction and the Pickelhaube sign. Magnetic resonance imaging (MRI) confirmed the presence of myocardial fibrosis, a potential arrhythmic substrate. The objective of this article is to emphasize the importance of echocardiographic assessment in identifying SCD risk factors in MVP patients and highlighting the complexity of this condition.
